In-office appointment only. New Patient Visits have to be scheduled at least 2 business days in advanced. It is your responsibility to verify with your insurance that we are in net-work. Please add your birthday, address and ph# when you set up your profile. Then text the office @385-429-3365 a picture of the front & back of your insurance card after booking. We will then send New Patient paperwork. We prefer for you to come into clinic for your 1st visit if at all possible. If you schedule an appointment & have to cancel or reschedule sooner than 24 hours before your appointment due to us not taking your insurance or you not scheduling appropriately you will be charged $150. To avoid this please verify your insurance is in network with us first before scheduling & make sure all New patient paperwork is completed before your appointment or come 20 minutes early.

In-office appointment only. This is for a brief appt. If you 1-2 concerns or are following up on a medication this would be an appropriate appointment to schedule. Lab results can not be reviewed during a brief appointment. It you have more to discuss then this then schedule a regular visit. Your provider will bill codes to your insurance based on the medical complexity of your visit not the price you see online.
